Registration fee for first semester of $50.00 is
required at sign-up. Continuing Students per semester is $25.00. Fee for
registration and materials are non-refundable.
Payment for Children's Curriculum:
Full
payment by cash, check, American Express, Visa, Discover or Master Card
Tuition
Fee: $400.00 Per Semester (14 Group Sessions)
Lesson Length: 30 - 45 minutes
Students Materials: $85.00 per Semester
Payment:
Materials, registration
and full tuition fee due upon new student pre-registration or fourteen business days before the end of continuing students'
previous semester.
At the conclusion of Fall Semester, tuition is due for the Winter/Spring
Semester to secure your student's seat in class. At the conclusion of the Winter/Spring Semester, tuition is due for the Fall
Semester.
WITHDRAWAL/ REFUND POLICY/ CHARGING
POLICY
Non-atttendance is NOT considered a withdrawal. Fees and
materials are non-refundable. Refunds will be given in cases of insufficient enrollment that results in the cancellation
of a class.
Prior to the start of semester, withdrawal is 45 days for Winter-Spring
Semester / 90 days for Fall Semester in written request by parent/student and must be acknowledged by Lozada School of Music.
Otherwise, there will be no refund. No exceptions. Paid books/materials are forfeited/non-refundable
and only released to attending/participating students with paid tuition.
Lozada
School of Music has the authority to charge credit card on file. For your convenience,
students will be automatically re-enrolled for the following semester upon advance paid tuition and fees.
MISSED LESSONS:
Practice assignments available via e-mail on
Word Document. All missed lessons because of teacher absence or weather-related school closing will be made up.
SCHOOL CANCELLATION / CLOSING POLICY / HOLIDAY BREAK
Please contact the school to verify the status of classes during times of inclement weather or other emergencies.
Note that the school does not necessarily follow the weather cancellation decisions or scheduled breaks of all other
local schools.
RELEASE
I understand that all students may be photographed and video recorded while at the studio or in performance
and understand that these pictures/videos may be used in flyers, advertisements, school website or social media without compensation.
I give Lozada School of Music full permission to use them. At all times, children must be escorted by parents/guardians
in and out of the building, up and down the stairs, good or inclement weather. Disruptive behavior during class may be dismissed
by teacher without refunds - No exceptions.

Young Musicians Course (Age 6+)
Keyboard Encounters Kids Course (Age 10+)
Keyboard Encounters (Adult)
AGREEMENT
The
signer understands and agrees to the above policy and accepts responsibility of ALL charges and fees towards the ENTIRE FULL
semester and any other fees that may be incurred.
